The Benefits of Alternative Therapy for Mental Health


One Buffalo Facility Is Leading the Way

Our bodies respond to how we think, feel and act. Called, “The Mind/Body Connection,” being stressed, anxious or upset affect us mentally and physically. For example, a headache or stomach ulcer might develop after a particularly stressful event.
In 1992 the National Center for Complementary and Alternative Medicine (NCCAM), at the National Institute of Health (NIH), began evaluating alternative methods of treatment. They found combining alternative methods of healing with traditional treatments has a positive effect on patients with a mental health diagnosis. A recent study by University of Massachusetts Medical School showed mindfulness meditation may cause measurable changes in the brain regions affecting memory, learning and emotion.
Here in Buffalo, BryLin Hospitals has been at the forefront in incorporating alternative therapies for mental health. Massage therapy: The underlying principle of this approach is that rubbing, kneading, brushing, and tapping a person's muscles can help release tension and pent emotions. It has been used to treat trauma-related depression and stress.
Tai Chi: Some people find that their spirits soar and experience a sense of release and inner peace from Eastern martial arts, such as Aikido and Tai Chi. Those who are recovering from physical, sexual, or emotional abuse may find these techniques especially helpful for gaining a sense of ease with their own bodies. The underlying premise to this movement therapy is that it can help a person integrate the emotional, physical, and cognitive facets of "self."
Pastoral Counseling: Some people prefer to seek help for mental health problems from their pastor, rabbi, or priest, rather than from therapists who are not affiliated with a religious community. Counselors working within traditional faith communities increasingly are recognizing the need to incorporate psychotherapy and/or medication, along with prayer and spirituality, to effectively help some people with mental disorders.
When BryLin was founded in 1955 it was primarily known for substance abuse counseling. Today, in addition to treatment for substance abuse, BryLin is the only private facility in Western New York that offers inpatient psychiatric services for children, teens and adults of all ages. Every May, Older Americans Month honors and recognizes seniors for the contributions they make to our families, communities and society. In keeping with the Administration on Aging theme for 2012 “Never Too Old to Play”encouraging Older Americans to stay engaged, active and involved in their communities, the Department of Senior Services seeks nominations for Erie County’s 2012 Senior of the Year Award, to be presented in May.

In April 2010, the museum was awarded a $100,000 grant from the Community Foundation for Greater Buffalo’s 21st Century Fund, which was used to hire ROTO Studios, an exhibit and experience design firm, to help create a master plan for the complete renovation and evolution of the museum’s science experience. With proper funding, the museum will open two science studios a year for the next four to five years.

The John R. Oishei Foundation last year awarded a $1 million grant to support the Buffalo Museum of Science’s master plan. The award provides funding for two science studios, Health Sciences and Earth Systems, scheduled to open in October 2012.

“The Explore YOU exhibit is the beginning of a major transformation at the Buffalo Museum that is built on strong partnerships and direct connections to the community,” stated Robert D. Gioia, president, The John R. Oishei Foundation. “We are thrilled to be part of this effort that makes science engaging and relevant to visitors of all ages. Congratulations to the Buffalo Museum of Science and Independent Health for creating a dynamic, exciting experience as the first science studio in the Master Plan!”

In addition to revitalizing the visitor experience, these studios will allow the museum to showcase science and research being done in Western New York, while encouraging area residents to pursue higher education in the sciences.

In order to help shape the Science Studios, educational partnerships with the University at Buffalo, D’Youville College, and Buffalo State College are underway. The museum will also showcase work from the researchers at Roswell Park Cancer Institute and Hauptman-Woodward Research Institute.

SUNY Buffalo Study: Constant Stress Linked to Memory Loss


STUDY BY SUNY BUFFALO RESEARCHERS REVEALS WHY CHRONIC STRESS IMPAIRS MEMORY AND MAY TRIGGER MENTAL ILLNESS


hronic stress has a more powerful effect on the brain during adolescence than in adulthood and now there's proof at the molecular level, according to findings published in Neuron by University at Buffalo researchers.

"We have identified a causal link between molecules and behaviors involved in stress responses," says Zhen Yan, PhD, a professor in the Department of Physiology and Biophysics in the UB School of Medicine and Biomedical Sciences. "It's the first time that the loss of glutamate receptor has been causally linked to the negative effects of chronic or repeated stress."

The UB research bolsters the emerging understanding among neuroscientists that the glutamate system is a key player in mental illness and, thus, is critical to understanding how to better treat disorders like depression, anxiety and schizophrenia.

Yan and her colleagues wanted to better understand the molecular mechanisms of stress, about which little is known. She and her colleagues had previously found that acute stress helps sharpen Now they have found that chronic stress has the opposite effect.

The UB research was conducted on male rats at an age that corresponds to adolescence in humans, a period when the brain is highly sensitive to stress. This is especially true of the prefrontal cortex, which doesn't fully mature until age 25 in humans and which undergoes dramatic change during adolescence. The prefrontal cortex is referred to as the "CEO" of the brain, controlling working memory, decision-making and attention.

In response to repeated stress, Yan and her colleagues found there was a loss of glutamate receptor expression and function in the prefrontal cortex. That loss resulted in a significant impairment in the ability of the adolescent animals to remember and recognize objects they had previously seen. The same cognitive deficit was not seen in the similarly stressed adults.

In the same paper, the researchers report that by disrupting the enzymes that trigger loss of glutamate receptor expression, they were able to prevent the cognitive impairment induced by repeated stress exposure. As a result, the UB researchers have discovered that there may be a way to prevent the detrimental effects of chronic stress

The research is especially significant because with some mental disorders, such as schizophrenia, onset typically occurs in late adolescence.

The research was funded by the National Institutes of Mental Health of the NIH.

Groundbreaking Heart Procedure Makes Buffalo General Medical Center #1

Something that never happened before in Western New York took place in January. Janice Savey of Olean and Robert Cecchini of Tonawanda, who suffered from severe untreatable aortic stenosis, underwent a new type of surgery, and they believe the outcome is nothing short of a miracle.

That’s because a new ground-breaking procedure called transcatheter aortic-valve replacement (TAVR), which treats narrowing of the aortic artery, is now an option for some patients. And Buffalo General Medical Center is the only facility in Upstate New York and West of Albany that has been approved to perform this complex non-invasive procedure.

According to the American Heart Association, more than five million Americans are diagnosed with heart valve disease each year, and up to 1.5 million of them suffer from aortic stenosis. In Western New York there are about 75,000 cases, and with an aging population, those numbers will continue to grow. Symptoms of the disease can include chest pain, shortness of breath, dizziness, and fainting. Approximately 50 percent of those who have it will not survive more than two years without valve replacement following onset of symptoms. And if a patient has other serious health issues, making open heart surgery impossible, TAVR may prove to be an effective option.

In a normally functioning heart, blood travels from the left ventricle through the aortic valve and into the aorta where it is carried through the body. Aortic stenosis occurs when the aortic valve narrows or fails to open and close properly, forcing the heart to work harder and harder. The extra stress causes the heart’s muscles to weaken over time, increasing the risk of heart failure.

TAVR involves the insertion of a biological valve through a small cut in the leg. Once delivered to the site of the  diseased valve, the new valve is expanded with a balloon and immediately functions in place of the patient’s valve. This allows for valve replacement without traditional open-heart surgery and while the heart is beating, avoiding the need for cardiopulmonary bypass.

Vijay Iyer, MD, is Medical Director of Cardiology at Buffalo General Medical Center, and practices with UBMD Internal Medicine. He attributes the selection of Buffalo General to the volume of cases they have, and their years of excellent outcomes. Although more facilities will likely open in our region, Dr. Iyer emphasizes the importance of patient volume and the need for a team that includes heart and vascular surgeons, anesthesiologists, specialists in heart imaging and cardiologists to perform each procedure.

“TAVR is a major milestone in the treatment of valve-related heart disease,” says Dr. Iyer, and an example of a great collaborative of Gates Vascular Institute and UB’s Clinical Translational Research Center. Located adjacent to Buffalo General Medical Center, the new center will officially open April 1.

For now, Dr. Iyer estimates the procedure will initially be limited to about 70 patients a year who meet the necessary criteria. It isn’t approved yet for younger or less-risky patients, but for those with more serious conditions there is renewed hope.

Roswell Park Researchers Publish Findings on Agent Shown to Enhance Immunotherapy


Conclusions from study of entinostat combination to be tested in kidney, prostate cancer clinical trials

BUFFALO, NY — Preclinical research by a team from Roswell Park Cancer Institute (RPCI) has demonstrated for the first time the way in which a class I histone deacetylase (HDAC) inhibitor, entinostat, appears to boost the effectiveness of cancer immunotherapy for treatment of some solid tumors. The findings support conclusions from earlier studies and also suggest that the approach merits further testing in clinical settings.

In a study involving models of renal and prostate cancer, recently published in the peer-reviewed scientific journal PLoS One, a team led by Roberto Pili, MD, Professor of Oncology, Chief of the Genitourinary Section and Co-leader of the Genitourinary Program, Department of Medicine, demonstrated the novel immunomodulatory effect through which entinostat, manufactured by Syndax Pharmaceuticals, Inc., appears to enhance the antitumor activity of Interleukin-2 (IL-2) for kidney cancer and of a survivin cancer vaccine (SurVaxM), developed at RPCI by Michael Ciesielski, PhD, and Robert Fenstermaker, MD, of the Department of Neurosurgery, for prostate cancer.

“Entinostat has an immunomodulatory effect on regulatory T cells, which are like brakes for the immune system and are present in many tumors,” Dr. Pili explains. “By suppressing these suppressor cells, entinostat releases the brakes and helps the immunotherapies to work better.”

The combination of a vaccine with approaches to deplete or suppress regulatory T cells, the researchers write, “represents a rational strategy in prostate cancer therapy” and one that “also has promising potential to be effective in other immunotherapies and in different tumors.”

Li Shen, PhD, a postdoctoral associate in the Department of Medicine, was lead author on the study. Drs. Ciesielski and Fenstermaker were also contributing authors, as were Swathi Ramakrishnan, Kiersten M. Miles, PhD, Leigh Ellis, PhD, and Paula Sotomayor, Department of Medicine; and Protul Shrikant, PhD, Department of Immunology.

How Music Changes Our Brains


Science is becoming increasingly interested in the relationship between sound and the brain. An expert explains

Music has never been more accessible. Just a decade ago, we were lugging around clunky portable CD players that weighed as much as a hardcover book and would skip whenever we made any sudden movement. Now our entire record collection (and thanks to new companies like Spotify, almost any other song on the planet) can fit into our phones. We can listen to music nonstop — on our commute, at work, at the gym and everywhere else we might want to. But what is this explosion of sound doing to our brains?
In their new book, “Healing at the Speed of Sound,” Don Campbell, an author who has written extensively about music and health, and Alex Doman, a specialist in technology in brain function, take an extensive survey about what the latest neuroscientific findings tell us about music and the brain. Although excessive noise has be harmful in a number of ways, music has been shown to improve children with learning disabilities, help elders feel more connected to the world, and even get people into better shape. It provides children with a “hook” for the brain’s memory centers, allowing them to retain more information, and it can play huge roles in modifying our moods.
Salon spoke to Campbell over the phone about the dangers of excessive noise, the importance of exercise music and why more children should learn how to play instruments.
As you write about in the book, there’s been a growing consensus in the last few years that sound can have some very strong effects on our overall health.
Science is beginning to ask very different questions than it was 20 or 30 years ago. Just 3 months ago, the World Health Organization published a 108-page study that absolutely shook me up. It argued that noise is the second greatest pollutant in the world today. Environmental noise affects cardiovascular disease, cognitive impairment in kids, sleep disturbance, tinnitus, not to mention just plain annoyance. If it’s too loud, whether it’s classical music, rock, whatever, it’s not good for us. And the numbers are just beyond me. The study said noise cost up 45,000 DALY [disability-adjusted-life years, meaning 45,000 years of "healthy" life worldwide are taken away by noise] per year.
I just returned from a week in New York City, and I have a little decibel app on my iPhone. On the subways it registered way over 100 decibels. When I was outside, I found myself covering my ears and needing to use my noise reduction headphones.
But as you point out in the book, the scientific community is increasingly interested in looking at the ways music can affect our brains. 
Our home, our work, everywhere around us is overloaded with sound. By selecting certain kinds of music, we can improve our lives and decrease our stress. Music triggers responses: If I speak in rhythm and rhyme, for example, another part of your brain will be [activated than the one that simply responds to speech]. Music can relate to emotions. It creates autonomic automatic responses.
What was surprising to me was, as you point out in the book, fast-paced music actually has real effects on your ability to work out. In one 2009 study you cite in the book, college students biked faster or slower depending on what kind of music they were listening to — and it made it easier for them to push themselves. 
If you put a rhythm behind your exercise, you don’t become as tired as quickly because of our natural tendency to fall into a pattern. If you want to do 100 pushups, you will start counting because you need that rhythmicity. The building of the railroads, for example, would never have happened as quickly without the use of repetitive work songs. One professor at the university of Northern Colorado has done extensive research into what they call rhythmicity and rehabilitation, showing that, for people who’ve had injuries, the use of rhythmic patterns, whether from a metronome or music sustains the ability to prolong exercise in rehabilitative therapy. It’s a technique called entrainment. The body creates the rhythm, and then the therapist uses the rhythm in a rehabilitative context.
From a neurological perspective, why is it important to listen to and play music from an early age?
The more participation there is with music early on — through singing and movement — the more it simultaneously activates multiple levels of the brain. If you look at the corpus callosum [of someone who plays music] there are more connections made between right and left sides. A child who is moving, dancing and singing learns coordination between their eye, ear and sound early on. And [the experience of participating in music education] helps integrate the social, the emotional and the real context of what we’re learning. There are studies that show children who play music have higher SAT scores, that learning to control rhythm and tempo not only help them get along with others but plants seeds for similar advantages when we get much older.
What about learning disabilities?
In many cases, a background rhythm will help and assist somebody with dyslexia or autism to speak and read in rhythm. Exposure to different kinds of patterns — high range, mid-range, low range, slow tempo, medium, high tempo — can help bring order to their thinking. In a 2001 study, one researcher found that brain activity changes when there is soothing music, and there is biological evidence that we can actually remove a great deal of the tension in frustrated children by exposing them to more soothing sounds. In the Journal of Complementary Medicine in 2007, there was a study that found that these children can channel more resources when they speak in rhythm and rhyme and when they play and listen to music.
What about the elderly?
Our hearing decreases radically after the age of 60, and often by the time we are in our 80s we don’t hear high frequencies and some sounds become more annoying and more confusing. Under different kinds of medication, tinnitus becomes more frequent. It’s a symptom, not a disease. By learning to tap a rhythm as one speaks with an elder, to use a drum, a simple hand drum, the size of a tambourine, to be able to translate and transfer the organization of speech and thought becomes much more effective. There’s a company called Oval Window that produces floors that vibrate, so elders they can literally hear better through the vibrations.
There are interesting studies on socialization. The more we lose our hearing, the more isolated we become. Music and rhythm help keep people wildly engaged in life. Gordon Shaw [a professor at University of California Irvine] has been a great leader in this field.  My mother had dementia for 10 years, and once I spoke to her in rhythm or played songs on the piano that my dad used to play for her when they were young, it woke her up.
Our lives are also much more deeply enmeshed in music these days. Now that music players are so portable, it feels weird to leave the house without listening to something on my headphones. 
If we live in a world where there is no silence or integration, we’re not as healthy as we could be. 125 years ago, all music was real and alive. We didn’t have radio and iPods, and now it’s just the opposite.
Right, and conversely we don’t have as much access to silence.
Silence is part of the brain’s pattern. It helps it reintegrate, like sleep, but we can’t shut our ears like we shut our eyes.
I also find that, now that listening to vast quantities of music is so easy, I get impatient 30 seconds into a song and skip forward to the next one for no real reason. 
I love exploring iTunes, but you only get 20 seconds of something to see if you like it. It’s like an all-you-can-eat food bar, but you need to understand the nutrition of it. If you’re changing music every 30 seconds, then be sure to have 2 minutes of real silence in there. It goes back to chew your food so to speak before you swallow it.
In the book, you describe something called a “listening disability,” or “auditory processing disorder,” in which children process sound incorrectly. As you point out, 2 to 3 percent of children suffer from it, but it’s often misdiagnosed as ADHD or hyperactivity. 
Listening disabilities were discovered by ear-nose-and-throat specialists early this century. They manifest themselves when students are not able to discriminate between sounds or they’re overwhelmed by sound. They may suddenly realize they misunderstand instructions, they have poor auditory memory, and are easily distracted, have reading difficulties and have language problems. Symptoms in auditory processing can affect our whole learning and communication system. The ear can cause difficulties within the other pairs of cranial nerves, like the vagus nerve that affects not only the diaphragm but also the heart and the larynx. If something is not right in our hearing, we may not be able to speak and talk about it in the correct way. I know of a doctoral student at the University of Edinburgh who is doing fine research looking at the auditory relationship on this point — we often don’t look at the ear as a source of reading and learning difficulties.
Clearly a lot of places are very noisy and there has been some argument that having  a TV or radio on in the background when you’re trying to do work is very distracting to people. Does the science back that up?
Different ages need different sounds. The brains of high school and college students are still developing and myelinating in such a way that sometimes sound allows them to focus better. Some of the newer studies have found that if someone is just having music in the background studying, very structured music like Mozart and Vivaldi and Haydn is better than pop music or nonstructured music. But we’re all different. For someone who lives in a home where there is a television, a microwave, the heater, as well as traffic outside, their mind can do nothing but work at filtering out sounds. For that person, silence can be quite frightening. And that’s where music can be used as a bridge. I’ve worked in classrooms of third-graders that have very little attention span, but if you give them noise cancellation headphones, they can suddenly hear themselves think.

New Hope for MS Patients in Buffalo

Multiple sclerosis (MS) is an autoimmune disease in which the body’s immune system attacks the central nervous system (brain and spinal cord). As a result, the protective myelin coating (sheath) on nerve cells which functions much like insulation on an electrical wire, becomes damaged, and signals from the nerves can become slow or blocked. Characteristic lesions called “multiple scars” can be seen on an MRI in a person with MS, which gives the disease its name.
Symptoms of MS can include weakness, numbness, tingling, balance and gait problems, problems with thinking, mood, and fatigue, bowel and bladder problems, sexual dysfunction, and problems with speech, swallowing or vision. MS patients often exhibit intolerance to heat making damaged nerves less efficient, bringing on symptoms.
There are 400,000 MS patients in the US, and over 2.5 million worldwide. The disease is of particular concern in Western New York where the incidence is significantly higher. This is because MS is correlated with colder climates and living above 40 degrees from the equator.  Higher incidences are also seen in Europe and in Caucasians of Scandinavian ancestry, although MS can affect any ethnic group. 
MS is thought to occur as result of geography, environmental influences, and possibly genetics.  Currently researchers believe low vitamin D levels may affect MS.  Symptoms usually occur between ages of 15 and 45 and two to three times more likely in women. Interestingly, MS exacerbations and relapses are less frequent during pregnancy, urging researchers to investigate the role of female hormones on MS.
Seven disease modifying medications (Avonex, Betaseron, Copaxone, Gilenya, Novantrone, Rebif, Tysabri) are currently approved for the treatment of Relapsing Remitting Multiple Sclerosis (RRMS). In September 2010, fingolimod (Gilenya) became the first oral medication to enter the market.  According to Malti J. Patel, M.D. who has conducted several MS clinical trials at the Dent, “This is a very exciting time for MS, and more drugs are expected to enter the market in the coming years, giving patients and physicians options to tailor MS therapy to specific patients.” 
But Dr. Patel urges patients to “stick with what works for you and work with your neurologist to choose the most effective medication for you, and one that you are willing to take regularly.” She says, “All MS drugs are effective, and have advantages and disadvantages.  Currently approved treatments can slow disease progress and provide improved quality of life, allowing patients to live more normal lives for a longer period of time.”
